Clam question

We have been keeping clams in our reef tank for about 3 years now.
We had four clams for the longest time but yesterday i noticed a whole in one of them, you could see down through the intake suction hole all the way to the sand. By 2am this morning it was dead??? Everything in the tank is healthy the clam has been in the tank for about 13 months alk and cal are perfect so is ph??? What could have cause this??? It was a blue maxx
 
i can see right through my clams, it shouldn't be anything to worry about.

hard to say what killed him, can you post some pics?
 
is it the byssal hole where the foot extends in and out or are they tiny holes along the bottom of the shell?..if they are tiny, say grain sized holes, I'd suspect pyramid snails as the cause of death.

Inspect your other clams for pyramid snails, they are tiny grain sized with a spiral shell. check a few hours after lights out for predation. If so, 6-line wrasses eat them or freshwater dip to kill manually.

Do a search for pyramid snails for more info.
