Clown Fish - Brook?

Hello all,

I need help figuring out what is wrong with my new clown. Had him in QT for 3 weeks without any issues. Moved him to my DT 3 days ago, and he started to turn south. He eats and swims around without any problems, but does have rapid breathing. No other fish in the tank, only 1 fire shrimp and CUC.

1st photo is 1 hour after being added to DT.
2nd photo is this morning. Notice part of the rear fin is missing. (not sure if the fire shrimp would do that or if it is part of the issue)

From what I have read online, it would seem to be Brook, but not too sure. Please advise on what it could be.

Based on the 2nd photo, I'd say that's definitely Brook.

Treat with Formalin. I've read conflicting things whether you just need to administer Formalin baths or dose Formalin in a QT. Whatever you do, do not use Formalin in your DT.
