More than likely not ich. It’s also hard to really see in the pic.
On a whole other note, his stomach is really pinched looking in the picture. For a fish that is eating well, this shouldn’t be. You might be looking at internal parasites. Have you seen any white stringy poop?
As for the pimple, I would keep an eye out for a bacterial infection. Are there any corals or anemones in the tank? Hypermelinaztion happens when a fish isn’t quite acclimated to the anemone and can resemble a blotch. Just throwing ideas out there.
