Clownfish line erosion help

I rescued a clownfish about 6 months back and he was in pretty bad shape. Stringy white poo, line erosion, rapid breathing etc. No signs of Ich though. Upon getting him home I promptly qt and treated with a smattering of my go to treatments, api general cure followed by prazipro, all while feeding 3 courses of NLS Hex Shield during the qt treatment. After this, all was looking up so about a month on I added him to my display tank. All was great over the next 4 months, it almost doubled in size, white poo was gone, rapid breathing subsided and the line erosion had fully healed. Well about a month ago the stringy white poo came back so I promptly fed him another 3 courses of hex shield and the white poo disappeared but about 3 weeks ago the line erosion returned. Its just strange to me as I have treated a TON of fish and I am not sure what to treat with next. My normal go to medications seem to have failed me... After much experimenting the treatment I went through above has treated everything I have encountered HLLE, ICH, Velvet, Brook, Flukes, Parasitic Nematodes and has always worked well and is gentle to boot! So I am at my end with how to treat this little guy, help????

So I don't get yelled at for not disclosing here is the full status rundown. Temp 79, salinity 34.9, Phosphate undetectable with hannah checker, Nitrates <5 but greater then 0, Mag 1480, calcium 430, alk 9.5, pH 8.1. As for the fish it is eating great, growing and personality is unchanged. As such I am hesitant to pull him out into another round of hospital treatments. Does anyone have a hint as to what I may be fighting here and how to treat it?? Please see the pics below it's the orange perc, and thank you all for your help!

Also his slime coat looks slightly discolored and the erosion has always been isolated to that one spot.

Oops forgot to throw in a blurb on tank conditions. As you can see I have been battling hair algae since I started the tank 8 months ago ( I am convinced my dry rock is leaching). Regardless parameters as of this morning are Temp 79.5, Salinity 34.9, pH 8.1, Nitrate <5 but >0, Phosphate undetectable (Hannah ULR meter), Mg 1380, calcium 420, alk 9.5. Fish is eating well with no personality or symptoms other then slime coat and line erosion contained to that one spot. All other fish are displaying nothing and all fat/healthy.

Are you using activated carbon? Some forms have been linked to HLLE.

Also, if you are talking about the missing white in the percula's head bar, that white will never come back.

Interesting I do use approximately a 1/3 of a cup of Seachem Matrix Carbon on a 29 gallon tank, which I change out every 3 weeks. And yes I was talking about the missing white, you confirmed my fears that this would be a permanent "scar" of sorts. Overall I am out of things I can do to help this little guy at the moment. I'm just glad he eats well and is growing quickly. I will be moving at the beginning of the year and use that opportunity to set back up my QT for a copper treatment.
