Clownfish thrown out of cave paralyzed with a cloud of dust.

Let me say, i have had some bad luck the last 6 months in terms of fish. Had a pair of clowns, the one clown i noticed one day started in a death roll, and then disappeared, yellowtail damsel a couple months prior, disappeared. Bought two fish, (the one died in less than 24 hours) and the other was fine for a couple of days (royal gramma).....disappeared. Leaving a hawkfish and the other clown, both are doing great. Until an hour ago when i couldn't find my clownfish. Starting to move rock, become sad, and then just stared at my tank in disappointment. A burst of sand comes from a cave and he comes floating out almost paralyzed.....still breathing, but barely waving fins, and not really swimming resting on the bottom. I have not heard any random clicks in my tank, but i am starting to wonder if there is a mantis shrimp, or or something eating my fish. Water parameters are all fine, I have some blue and green acros that are growing great, along with an enormous hammer colony. Its a 29 gallon reef tank. Does this sound like a mantis shrimp? Despite never hearing any clicking or anything.
