They toss their heads for a few reasons, primarily due to stress or starvation. As noted, if the worm is still in its tube, it will probably grow another head, albeit a smaller one. Make sure you are supplementing your tank with phytoplankton or some other marine snow/micro food to make sure your coco or duster is getting fed.
A long time ago, my duster jumped out of its body. Im not sure why, but It lived outside its shell for like 2 months. it just lived in my sandbed. lol.
