converting crushed coral to DSB

In a final attempt to lower my nitrates, i will get rid of my bioballs and CC. However, i am not quite sure how to do it. I plan on doing the DSB first. What is the best method for doing this? I need very detailed info since this could be a possible tank crashing experience. I have many questions:

What should i put the live rock in while doing the switch?
Do I need to get all the fish out? Can the fish withstand the probable sandstorm?
If I need to take the fish out how soon can I put them back in?
I was told to use a few handfuls of the old CC, would people agree?
I was planning on doing 4 inches does that sound ok?
What type of sand should I use?

I'd like to hear about this, too. I currently have two small tanks (12g and 3g) with CC and am setting up a 56gwith a DSB.
