Coral Beauty Problem!

We've owned a beautiful little coral beauty now for about 6 months. Been doing swimmingly in our nano tank but this AM I see him swim through the rock with a big white patch where an eye should be... after feeding he decided to come out of the rock and it is his eye! Not glazed over, but the whole eye is pushed out like some gold fish. What would have caused this and is there anything we can/should do? There's a chromis, pair of maroon clowns and a fire fish, plus a purple sea star, short spine urchin and a small Zoo Frag. All have been in there for ages now with the exception of the star whom was placed in the tank 2 days ago.
 
could just be he banged the eye.

if so do a water change and check all your paramaters and post them.

but look out for signes of bacterial infection fin rot red streaks the other eye going.

it this happens treat with anti biotics.
 
Thanks Andrew. I just did a partial water change before we moved the tank (last week). Nitrites are 0, Ammonia is 0, pH is 8.1 (if I remember correctly), and salinity is with in the normal range and the nitrates are at .2. We've been trying to get them down to 0 but haven't had any luck with that :(
