Coralife 9W 6700K lamp

Purchased 2 and installed into my 2x9W mini aqualamp fixture. One fired up fine; however the other one did not.

I installed 2 other bulbs into the same ballast (10K and 2 actinics) and all fired up fine. How do I go about requesting a replacement for this one? I purchased it last week and just received it in the mail 2-3 days ago? I don't have my packing slip; however, I still have my original receipt (pdf file emailed to me). Please advise? Thanks.
