Crocea clam is sick or being picked

Ive had a blue crocea clam for aprox 6 months now and its not looking too great. Its a 125 gal sps and lps tank, all the levels are fine. I have 3 tangs, a pair of clowns, and a Harlequin tusk. I have it on the sandbed, since it never stays on the liverock when i used to put it there ans its under a 250w mh. Ive noticed the last few days that there is some scaring on the lips that is a lighter color and in these area's the skin isnt smooth but has wrinkles. Also ther is a small hole on the lip that goes all the way through, alittle bit bigger then a pin hole. but the rest of the clam has maintaned its color and is still very bright. I feed every few days with plankton. Also ive had all the fish ive listed about, including the Harlequin with thsi clam for almost 8 months if not more and have never had a problem, so i dont think its that. Anyone have any ideas on what the problem might be?

what species of tangs??

water parmas??

did you look for pyramid snails when you got the clam


a picture is very important for diagnosis.

i have a powder brown, and 2 unicorn tangs

salt is at 1.026
calcium is at 410
posphates are untracible since i have a reator

i just have turbo and Nassarius Snails maybe 1 or 2 hermits walking around

didnt look for any snails on it when i got the snail but ive had it for along time and nothing has ever bothered it
