Cupramaine Treatment - Not Seeing Immediate Improvements

I have (what I think is) an ich outbreak, and one of my fish, my Kole tang, also seems to have velvet. I was able to pull it from the tank and throw it in the QT. After my initial dose of Cupramaine, he looked a little better (the "velvet" fell off shortly after, etc.). However, I just looked at him now and he has cysts all over him again. Is that typical in the first few days of QT, even after dosing with Cupramaine (I haven't used it before)?
 
Are you dosing the correct amount? Saltwater requires slightly higher dosage than the label suggests as I can recall. Here's a q&a suggesting 4 drops per gallon for saltwater fish. The label says 20 drops per 10.5 gallons but that's not enough per Seachems website. But the post itself is confusing. It says correct dose is 2 drops per gallon then goes on to say 4 drops to achieve final concentration. http://www.seachem.com/support/forums/showthread.php?t=3998
