It appears to be a condylacts. I've experience this scenario with this particular species. I wouldn't be too concerned. Provide appropriate water quality and lighting and you should see success.
They do that when they're too far from the light. If you could raise it up in the tank, you might have more success with it. Condylacti anemones are fairly hardy with water conditions, they just need sufficient light.

I disagree- what we see in the picture (broken/twisted tentacles) is NOT normally associated with an anemone not getting enough light. Usually indicates a fish chomped on it. Could be something else.... but not the lighting.
