Dead anthia, black spots IN gills

I've had 3 lyretail anthias in an established QT for a week now. This morning, one that was eating fine last night, was lethargic, and was not using one fin. Reddish around the fin. I was able to hand pick him out of the tank, so know it wasn't going to make it. Put the body under a microscope and looked at the gill, there were a couple of brownish patches that looked like scabs and a few thick black spots. The other gill looked good and red.

I got them from from Live Aquaria, matched sg and floated for temp before releasing into QT tank. No meds to date. SQ is 1.020, ammonia is 0.

I'm at a loss here. Any ideas?

Hey all, can't believe no one has any ideas. Its very frustrating to lose a fish that was so healthy. The other 2 are doing well so far, but would like to know if I should treat with anything.
