Desjardin Sailfin Tang has grey crusty patches around Eye.

This fish has been in quarantine for Ich. I am using copper sulfate for that. In addition I have been using Rally and the grey crusty patches have been shrinking dramatically. The Patches are responding quite well with Rally but they are still there. They appear white in the photos from the flash.

What is this disease?

Should I be doing anything else?

How long should I keep him in Quarantine?

First photo is 5 day into treatment, Second photo is yesterday.

It does not look like hole-in-the-head disease.

Really not sure what that is.

How is your water quality? Any chance of voltage leaking into the tank?
 
I just measured 0.16 volts DC from the sump water to ground.

Is that significant?

Do I need a grounding probe or find and replace the culprit or both?

Water quality as follows:

pH 8.3
Nitrates 20
Ammonia 0
Calcium 380
Alk 11
Sp Gr. 1.023
temp 80F
phosphates 0.75
