Diatom problem

So I currently have a 55 set up. 6 month old tank. Tank went through normal cycle, then diatom bloom. That cleared up by month 2. Battled a little bit of cyano a month ago. Got it out by sone rowaphos and water changes and manual removal. Anyway now at month 6 the diatoms are back. The pictures show my frag rack that I put in about 2 days prior covered. Flow is good I think. Got 2 800gph, 1 530GPh power head and running a hob filter with just some floss and rowa. One power head is directly pointed at the worst diatom spot and no change.
Lights are leds, and bio load is low, 2 fish. Feed half cube mysis daily. I also have some gha if that says anything. And a my fighting conch is kinda useless.

My only thought is the sand got stirred up pretty bad the other night when a power head fell. Had a sand mountain.

Nitrates? Phosphates?
