dips which one?

i did dip all my clownfish with formalin.
i always acclimated all the fishes by dripping, for 30 mins. then i take out 80% of the water replace it with my tank's water. then i add formalin and air stone wait 30mins then put them into some clean water from my tank wait 5 more mins and put them into my tank. but most the time the clownfish will ended up with not eating and swim at one spot and found dead couple days later.
are there something i did wrong?
 
I would definitely slow down the process you're using.

I would acclimate the fish to their QT first. Let them settle in a couple of days there. That initial acclimation must be done slowly and properly. I would perform a FW dip at that acclimation time, according to guidance found in various places here on RC or by the way I prefer to do it:

Let your fish settle into your QT a few days before taking any other treatment action. Acclimation can take days to complete. They should start eating at this point. After they start eating in your QT, then continue. . .

Then I would setup a separate formalin bath as recommended in that article I referenced. Then I would perform those baths, as recommended, with the fish in a QT.

one of the false percula is not eating and swim under the rock.
it doesnt look discolor and it did eat couple days ago.
i did a formalin bath yesterday. what should i do now?
 
How many formalin baths has this fish had?

When did you acquire it? Is it in your display tank?

Are you putting it in a formalin bath because you found a disease or just as a preventative measure?

What was it eating?

My primary concern is that you're not doing a formalin bath like in Terry's article I referenced. Formalin can harm a fish if the bath isn't controlled closely and done just right.

it ate cyclopeze and brine shrimp couple days ago.
it's in my nano/QT for now
yesterday i found it not eating and swim close to the surface. so i did a formalin dip(first time)
i did 2.5 G with 1.5ML formalin.
 
A fish that began to eat then stopped is a concern. There is definitely something wrong with it. But I'm not comfortable to giving it a formalin dip based upon that it stopped eating.

I'd focus on water quality issues. Test the water it is in. What is the Ammonia level, Nitrite level, pH, salinity, and temperature range.

Perform a water change if even there is a hint something isn't right with the water, or just do it anyway. :D

I would not give the fish any more dips. I'd focus on getting it to eat and that think you can best help it in that category by focusing on water quality.

When it does eat, remember your fish is an omnivore. It expects meats and vegetables in its diet. Check this out for more info:
