my yellow tail damsel has something on his head... dont know what and thats why im askin: could some1 plz ID this and tell me how to fix it? thanks in advance

BTW hes eating and swimming normal, and his other side is fine.
its seems like his scales are coming off...

is the fish rubbing on some rock or somthing? how long have the discoloration been going on?

Looks like he got in a fight with someone. If it were my fish, I would probably try Melafix. But get a few more opinions.
