1. How old is this aquarium?
This yellow flanked wrasse has been in my quarantine tank for two days and the tank has been cycled
2. If less than six months old, what is ammonia level?
Ammonia is at 0

3. What is SG of this aquarium? How measured?
sg is at 1.024 measured using a refractometer

4. When was the last fish added to this aquarium?
2 days ago in my quarantine tank

5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How?
still is quarantined and for 2 days. Have not treated for anything as of yet.
6. If you are using a copper based medication, which one? How often do you measure level? When?

7. If you are using hyposalinity, how did you calibrate your refractometer?
No

8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color?
Looks like one white little dot on the dorsal fin. When i first saw it i thought it was just some detritus ontop of his head but it didnt go away and ive seen it scratch itself on the pvc pipe a couple of times. Not that frequent but it caught my attention the couple of times its done it.

9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?
Looks healthy and swims all over the tank. Scratch itself every now and then on the pvc pipes.
10. Is the fish eating? What?
Yes. Selcon and garlic soaked brine and mysis, and also pellets.

It's a virus.

Do a search on it to see if it fits with what you're seeing
 
I cannot tell from the picture (my ability to diagnose from pictures is terrible). The scratching concerns me, however. How is the breathing rate?
 
The spot is too big for ich, I think its lympho too. This wouldn't cause scratching, I'd do tank-transfer to eliminate possible ich and watch the fish for other parasites during the QT period. Prazi-Pro, after tank transfer, should be done too. PP is a wormer, very safe & Easy----it should be used on all new fish; IMO & IME. Flukes in the gills can cause scratching (so can real deadly parasites) and the PP will take care of the flukes.

I've done a search but havent seen any real good pictures to help ID. It doesnt seem to be anywhere else. If it was ich there would be more than one spot i am guessing?

His/Her (havent named it yet) breathing rate seems normal. Its very active and eats like its never been fed before.

So for now I should wait until have a good 14 days clear to do the Tank transfer method? I am going to be out of town in a week so starting that method now would probably wouldn't make sense because of the interruption correct? Can i do the Prazi-Pro treatment for now since i cant start the TTM?
I have this bottle of jungle internal parasite guard, im guessing it contains Prazi-Pro (not currently at home to check the bottle.) Is there anything else i can do? It's looking pretty healthy besides the occasional scratching of the pvc pipe. Can it fight it off it if it stays eating and active?
 
It needs to contain Praziquantel. No harm in starting it but two treatments would be desirable.
