dying hammer coral?

I posted a similar thread in reef discussions, things have changed since the other day. I have a hammer that for the past few days has not been inflated. The some of the 'tenticles' are deflated while others are retracted. There is a vermetid snail on the base of the frag. I have been observing it for a few days to be sure the web is not touching the tenticles. Last night, I observed shirvled up pieces of the hammer on the rock near the frag, as well as on the sand bed and suspened in the water colum. I have searched for information online but have not reached a conculsion. Can someone here tell me what's up with it?

So many things could be wrong or so little. Hard to just guess
 
Like the previous poster said there are a lot of issues that could be causing your problems. I'll assume you don't have any fish irritating the coral? Is it possible the Hammer is receiving too much flow? A picture would be helpful.
 
The water parameters are as follows:
specific gravity 1.024
temperature 80.0 degrees
nitrate 0ppm
nitrite 0ppm
ammonia 0ppm
calcium 450
phosphate .01 mg/l

I use four t-5 h.o. two 12,000 k and two actinic blues. I run the blues twelve hours and the 12,000k eight hours. I feed usually feed it once every week, a frozen brine shrimp. I had placed the coral in a moderate flow zone closer to the top center of the tank. The coral had been doing well since I brought it home in the spring. I took several pictures of the coral, however, we just purchased a new computer and I cannot find the port for my memory card.
