Why did it puff up? Is it stressed?

I dont think so. It is by far the largest of the 4 baby fish in a very large tank.
It puffs up once every two months or so that I see, and its always in that exact area every time. I think it just sees itself possibly. I probed the area for electric current just to be sure, nothing.
Water quality is perfect too, so i dont know

Usually they puff up if their stressed, and than they can clog their throat and die o_0
