Fish dying, need help!

Over the last week I've now slowly lost 3 fish after not losing any fish in close to two years. They go from looking fine, to hiding for a day to head the next.

Now my beloved powder blue is sick.

I can't see any Ich on him but his fins are looking really bad. The others fish that died didnt have anything on them that looked wrong before or after they died.

No fish have been added or any changes made to the tank other then a couple frags added in the last couple weeks.

I put the tang in my acclimation box to get some pics and to either do something for him or if I need to just take him out.

What is it and what can I do????
 
Question: Are those dark splotches on the body and on the tail & bottom fin? Are is it just the camera or shadows?
 
Do the fish have a history of disease?

The PBT is painstakingly skinny. Has it been eating well? If it has, then it probably has internal parasites that are feeding on the food and not letting the fish have any. Over time, this causes malnourishment and the fish slowly dies from lack of nutrition. How long have you had the fish?
 
I have had the fish for 3 years. Its always eaten like a pig but stayed skinny, not not as skinny as it is now but still skinnier then my other tangs.
 
Have you checked your tank parameters? Mainly, ammonia and pH? Also SG.

It looks like your PBT has multiple issues right now. I would first test all your water parameters and if that all checks out, I would QT and treat with antibiotics (probably Furan-2). The tears on the top fin could be rips or possibly fin rot. Also, the "puss" on the tail doesn't sound good. So IMO antibiotics is a good place to start. However, this doesn't completely explain why you've lost 3 fish so far. It's rare for an infection to spread from one fish to another, but it's not completely unheard of.

While this does not explain the loss of fish, you have an internal parasite issues that's been plaguing the PBT, and possibly the other three fish that died. If the PBT has always been skinny, it has not been getting nutritions and was just slowly wasting away. Its immune system was probably shot due to years of malnourishment, and this would make it prone to getting sick.

You will need to put the PBT into a QT and apply prazipro (2-3 treatments) to deworm the fish. Then slowly bring it back to full health. Bobab0ey's suggestion of antibiotics is good too if infections are present, but I would deworm him first as he's probably in a weak state.

Good advice. After deworming, I would suggest PE mysis as a protein/fat diet coupled with veggies. Multiple feedings per day.
