If the fish is scratching continously, (like 2 times per 5-15 minutes) it probably has an external parasite. If the fish just scracthes a few times a day, you should not be concerned. Eventhough scratching could mean a parasite invasion, it could also indicate (believe or not) diarrhea.

Mine do this too, and have done this for awhile. Mind you, awhile for me is a couple months because my tank is only a couple months old. I have a wrasse, a few blue chromis and a sailfin tang that all do it, on the same rock. No visable signs of ich ever, so I figured it was just a 'marking territory' action... (either that or they touch it for good luck since its a rock all alone in the bottom of the tank)
