Flukes, Ich, Brook, or All the Above?

Tank info: 300 gallon, dry rock
Livestock: Regal Tang, Naso Tang, Chromises, Cardinals, Azure Damsels, Firefish, Blood Shrimp, Cleaner Shrimp --- no corals or anything else.

About 1.5 months ago I treated a clown for Brook and he has been fine ever since.

A few days ago my Square Spot Anthias died, it did not appear to be eating the day or two before. The next day my Flame Angel died who was eating fine and was swimming around happily. My Orchid dottyback also died at the same day, but earlier seemed to be hanging around the bottom more and less active. Last night I found my Royal Gramma as well. None of these fish had anything visible on them.

Yesterday I noticed the following from my Regal & Naso:

Cloudy Eyes, and a sudden break out of white sugary spots. Also hiding around the rocks, and barely swimming. The Regal was eating a little, but the Naso wouldn't touch anything.

I figured Ich, but the cloudy eyes and lack of appetite I thought might be Flukes. So I did about a 5 minute RO water dip in a bucket with a tsp of Baking Soda, and temperature matched water. The regal appeared to tolerate it ok, but the Naso was on it's side barely moving almost immediately so he only got about 3 minutes.

Here is the Regal this morning:

White spots appear to be mostly gone, and was swimming around almost normally this morning. Didn't really eat much, but really perked up with food in the tank. The Naso is still hiding in the rocks, not interested in food, and I couldn't really see the white spot situation.

My two questions:

a) What have I got on my hands here?

b) How should I treat it?
I'm considering running my display tank in Hypo for a couple months (guessing I probably have to remove the shrimp)

I also have a 56 Gallon for QT, and a 20 Gallon for QT or Hospital purposes.

Any help would be greatly appreciated!

This is a bad case of ich. Its common for it to disappear, then reappear X10. All your fish need to be treated in a HT and the DT left fishless for 8+ weeks. Have you introduced anything recently; fish, even LR, coral, anything wet? You could remove inverts and hypo the DT; but be prepared to do a lot of WCs as die-off from the life in the LR & substrate produces ammonia.
 
Just got back from Christmas for 3 days, and returned and found the Naso has perished, and a bicolour Chromis. :-( The Regal is is hanging on but is pale, got very skinny, and is gasping at the surface. I had some Amonia 0.5, which I immediately put Amqel in.

I think the ich came in with the Regal. I thought I was ok as he had been in copper at the LFS for almost 2 months. I had to learn the lesson the hard way.
