Foxface and Kole Tang dead... Ideas?

So I lost my onespot foxface on sunday night and came home today, tuesday, to find the kole tang dead. The kole showed no outward signs even taking food monday night and some this morning. I have a 6 line and 2 clowns in the tank that seem fine, but I'm wondering what could've gotten these two. The foxface had a rough couple of days before he died and had a weird spot on his torso that discolored around it (see pics). The tank is a 120 gallon, i run carbon and gfo. I've done a 20 gallon water change weekly since September and thought everything was great. Corals are growing better than they ever have, but now the damn fish are dying. The kole had been in the tank for almost 3 years. The Foxface was a new addition - I'd had him for 3 months, so I'm wondering if he was sick and brought something in with him. My LFS keeps them for 2 weeks, and I kept him in my QT for another 3 before I added him and seemed fine until friday. Over 2 full months of normal behavior. Ate everything I put in. Usually mysis or formula 1 pellets and an algae sheet once or twice a week. Salinity is right around 1.026, alk is around 9 and calcium is around 395. Been trying to raise that a tad, but wasn't going crazy about it. I haven't tested for nitrates in a while, but I'll be doing that later, but with my weekly water changes I wasn't terribly worried about that. I'll attach a couple pics of the foxface at the end to see if anything comes to mind for anyone. There seemed to be some marks on the foxfaces tail you can see in the 2nd pic... It didn't look like ich to me, but now that I've lost two fish, i'm worried about something knocking them all out. Anyone have any thoughts, or things I can look for or test for?? Thanks and sorry for the rambling.

prolly not ich you would of seen white dots, 3 months into owning is awfully late for a fish to succumb to a sickness with no prior signs, especially with you QT'ing. So I would rule out him being sick initially as well. Odd case here.... Nothing else is showing any distress?

with those red dots on the foxfaces gills im going with bacterial infection, you might get some better answers in the fish disease topic

Not so far... The clowns and 6 line seem fine. I'll be eyeballing them more than normal over the next few days, but they seemed perfectly active and eating. I thought the Kole seemed less active than normal, but he came right to the top when it was feeding time and definitely took some food. Now that I'm thinking he / she did probably eat less than normal. He'd take some food and then head into the rocks, but I figured any eating was good. The two that are dead were my herbivores... They would both take anything I put in there, but they were the only two that were interested in the algae sheets... wondering if that could have anything to do with it. Algae sheets don't go bad do they?? I know I've had the same pack for a really long time, but never thought twice about it.

And good call on the dots...I honestly didn't notice them. I just chalked it up to him being dead, but they're definitely not there in the alive pic.

And I did give the disease forum a try when the foxface kicked it, but got no replies... I'll update my post in there and mention the spots.
