Frogspawn Help!

I came home from an evening class and discovered half of my Frogspawn detached from its skeleton, still clinging by a small thread of tissue. It is in a moderate flow area. I have had the frag for going on 3 months now. My parameters and livestock will follow. I don't suspect it was a fish because it wasn't bitten. I suspect it's due to a nutrition deficiency but I don't have enough experience to know. Any insight would be much appreciated. I'd like to be able to prevent a recurrence.

Did a 30% water change immediately following discovery, using Red Sea Salt.

Livestock:
Blue Spot Goby
Magnificent Foxface
Melanurus Wrasse
Christmas Wrasse
2x Cleaner shrimp

75 Gallon Tank
 
What are your Mg, dKH, and CA levels? Also your SG is a bit low. 1.026 is better for LPS. What is your tank temp. and type of lighting, also?
 
What he said, its called polyp bail out and can occur if its in too much flow or not enough cal, mg, alk, or ph is off
