Frogspawn tree issue

I have had a branched frogspawn tree for about 3 months now and it was doing great, but this past week it started closing up and today I noticed one of its tentacle floating around in the tank. It has 3 heads on it and when I took a closer look it looks as if there is a small section that has some missing tentacles on the head that has closed up the most.

Any ideas what might be the issue?

Nitrate
Phosphate
Alkalinity
Calcium
Magnesium
Temp
PH

How old is the tank? Types of fish?

The more information you can give the better when trying to figure out a problem.
