Thought they were not reef safe?
If so, is he safe in there? Or does he graze?

No, there is not or ever was lunar wrasse in that reef, imo definitely not reef safe. And if I am correct they need a sand also to thrive.

You are correct about the wrasse proffering sand and being reef questionable. For sure they chow inverts.

What's the lyre tail above the purple tang in the last pic? Niger trigger?

That's actually Red Tail Triggerfish (Xanthichthys ringens).
