1. How old is this aquarium?
Roughly 3 months old.
2. If less than six months old, what is ammonia level?
0
3. What is SG of this aquarium? How measured?
I think SG is referring to the salt levels? Not sure but I use water from my local fish store to do 30% water changes every other sunday.
4. When was the last fish added to this aquarium?
About a week ago, it was a clown who made his way into my sump and has sense been sent to fishy heaven. I tried to add a clown due to one of my 2 clowns passing so 2 clowns total have made it into my sump I still have 1 from original stocking. Replumbing is in effect due to this issue, all fish are currently in a holding tank for about a week.
5. Was it quarantined? If so, how? And how long? Was it prophylactically treated? How?
No quarantine, simply let the fish bag sit in tank for about 15mins and put him into aquarium.
6. If you are using a copper based medication, which one? How often do you measure level? When?
No copper, I'm very new to the hobby.
7. If you are using hyposalinity, how did you calibrate your refractometer?
Not using one.
8. Please describe in detail, the appearance of the fish? If there is one or more pimples, are they lumpy? What color?
Fish visually looks fine, sense his mate passed he has found a new spot in tank to host, back corner of tank. I recently drained my aquarium due to needing to relocate tank and need to replumb the tank. Since transfer to holding tank a stringy like substance has shown on the lower side of fish.
9. Please describe the behavior of the fish as best you can. Is it acting reclusive? Is it always up towards the top of the aquarium? Is it avoiding light? How active is the fish?
Sense loss of mate fish has moves to back lower corner of tank not very active.
10. Is the fish eating? What?
No, I'm feeding pellet based food, I will be purchasing a frozen food on way home from work today and trying to feed that tonight.

Here's a few photos of my temporary setup while I undergo construction on the main 55 tank. It's a 20 gallon temp tank hoping to use at a quarantine tank in future.
I'm very new to the hobby and will appreciate and help.
