Help diagnose Sick yellow tang with pic

She has been with me for a little more than 5 years without any illness, last week I noticed some swelling on what I guess would be called the snout? Between the eye and mouth (just on one side) I seen the cleaner shrimp picking at it that night and the next day the swelling was gone.

Today the same spot is very swelled and there's what looks to be a wound there. I noticed her scratching it on the rocks and sand today also so I'm thinking that could be where the wound come from? Heres the best pic I can get the wound is right below the eye and the swelling starts there and ends at the mouth, thanks for any help.

Who does she have for tanks mates? Anything in the tank that can sting her (e.g. rabbitfish)? any nems? Any recent additions?

She looks nice and plump - she still eating?
 
I dont have any anemones but I was thinking it could be a sting because of the swelling, maybe?

She eats plenty all day no change there. The wound has lost it's redness and is just white now.
