Help identify tank problem??

Hey guys. I haven't been on the forums lately but I need some help. My 75 gallon (aggressive) fish only tank is starting to show some problems and if it's a disease like ich or something I'd like to fix it asap. I have a big lionfish, a banded eel, and a panther grouper. For the past month or so the grouper is showing loss of his fins, they look like they are withered away and getting worse. He sits sluggishly on the bottom of the tank all day long and doesn't have much of an appetite. I'm posting pics. But I have no idea what's causing this. All of my tanks parameters are fine . Please help I don't want him to die from something that I can fix!! Thanks guys.

The lion has some white spots in between the spikes on his fins but I don't know if that's part of his pattern or if it's something else?

Those are some fast-growing fish. Maybe they are outgrowing your tank.

They also eat a lot, usually live food. Sometimes the availability of live food leaves nutrition wanting for salt water fish.
 
How long has your tank been set up? Your rock work looks very new, as well as the rocks in the bottom (maybe sand??). Can you post your actual parameters and which test kits your using? Any info on filtration?
