HELP! Is nem about to die or split?

Hello today I woke up and noticed my BTA looking pretty crappy. Three days ago he decided to move towards the back of the tank from his happy spot he's been in for months. He was really deflated today and decided to attach himself onto a rock and stretched to the glass trapping in a margarita snail under his foot. I gently took him off and put him in a holding container closer to the water flow ( attached to the bottom within ten minutes) because my clowns are like molesting him or something. He is all pinched up in the middle all around and his mouth is open a bit and has a bump of it sticking out on the one side. Also I noticed a very tiny hole on his foot directly under the mouth. Does it sound like a split I never had a nem split on me before.

Looks better then yesterday. He is still scrunched up tight from his mouth in a line down the side. Very strongly attached to the bottom his foot print is roughly 3 inches around. He's pretty deflated.
