Help me Id this disease

For the last week my yellow tang had a few white spots on him, larger than standard ich. It seemed to go away on it is own but next my kole and flasher developed the same condition. All fish have been eating well, but the kole has started to be covered in a film that looks like fungus.

Well this am I found him floating so I took a few pictures, This is the a new tank around 2 months old, all my fish were fine in my old system, no new livestock

A time line for the last wet item added would be beneficial, but initial appearances seem to be velvet.
 
I addded my fish around 3 weeks ago, with in a few days the Yellow tang showed signs of this, then my wrasse kole and mandarin showed symptoms after a week. The YT shows no signs of the disease now, but the in the last 24-48 hrs the kole started to really get covered all while eating normal. Finally this morning I saw him upside down, and with in a few minutes stuck to a powerhead dead.
 
Any comments on how to treat this, I have started relocating all my fish to a QT, lost a male mandarin and a flasher wrasse as of now.
 
I got home, my female mandarin was dead in the QT and my blue flasher was not looking good. I dipped him in a freshwater bath with prime for 5 minutes and replaced him, alot of the spots fell off, but a few were still present that I photographed

He is alive but sitting at the bottom, he is pale but no film over him, also he is missing a few fins on his side.





The patches show the missing scales and a few dust pieces show the parasite, still not sure what it is?

The death toll is one kole tang, one male mandarin in the dt, female mandy and carpenter flasher in the QT. I still have the yellow tang in the DT, but he looks great even though he was near death last week....
 
It is still necessary to treat the tang in QT with copper (my choice) or chloroquine phosphate (hard to source/issues with quality).
 
Velvet. Six weeks fallow will eliminate it from your display tank.

I want to leave the tang in the DT for a week or two till I get my QT running proper. I figure he is healthy now he is best waiting for a stable QT.

I figure Ill get him in the tank then after a week start copper?

Sound like a good plan?
 
I would set up QT with a sponge filter air pump and some of that instant bacteria starter. Velvet kills very fast. He probably wouldn't make it a weak honestly.
 
Well he Had signs of it and since seems much better, I am going to QT him this weekend. Since the other fish died in there, I want to clean that QT well, any suggestions?
 
I would be very careful. Velvet is nothing like ich. A tank could have ich for a long time, but nutrition could keep it from causing a ton of damage. Velvet hits hard and fast. You need to get that fish in a qt and get treating like now.
 
Fair enough, are we sure its velvet. Im not opposed to treating the fish, I just want to make sure the QT is able to handle him and clean from the previous deaths.
 
Sorry for your loss. Formalin is not a recommended treatment for velvet (Amyloodinium ocellatum). Copper or chloroquine phosphate are the recommended treatments. As indicated above, you need to treat your remaining fish soon, or they will succumb to the disease.

Thanks chris, I read that Foymalin is a good treatment to give the fish relief, I have copper on order with a test kit (couldnt source it locally) but this was the last fish in that tank (the rest are dead). I have a 20gal with my clown fish and a corris wrasse that look fine, they were with the other fish but the disease happened upon moving the new 90 gal tank.

So going forward 6 weeks fallow (is this enough) and start QT new fish, can I use water from the infected DT in my QT or would this potentially present a problem even though I am going to treeat all new additions via copper?
