The fish is extremely emaciated...it's starving. I also see a few spots that look like crypto, but the light patches I'm not sure about. I suspect the stress of being starved brought on the other issues.

Unless you get the fish eating, it's doomed, I'm sorry to say.

How long have you had it? What is it eating (or is it eating)?

I fed it with salt water "Moina mongolica" (I watched) last Friday when I realized the disease was developing. It just become very inactive since the next day... It usually feed on the small insects in my tank.
I brought it 6 weeks ago when I found my tank is full of small animals.(pods etc)
I also culture those moinas for it but only fed twice...(Yes it is eating. It has this emaciated looking before I brought it, for the retailer's tank is too clean...)
