Help my coral!

I've had this bad boy for about a month and he's been doin great until yesterday. As you can see from the picture he's not doin too hot. The bottom polyps have shrunk to the skeleton and one seems to be dying. Water params are fine and all other coral is fine.

No recent changes. About half a week ago I did a normal water change. A week ago I added a new HOB skimmer. And maybe 2 weeks ago I added Isaurus tuberculatus.

I bought some coral cleaner dip just in case there's something going on under the polyp. Will use it tonight

Okay *** is this? Lol this is scary looking and definitely not the normal tentacles. This came our after the dip(hence why it is all retracted to the skeleton). Well many flatworms came out of the dip and also I found a hydroid next to the dead tissue, most likely causing it. But can someone please identify what this white wormy looking thing is on the far right polyp? It gives me the creeps. Thanks.

It is probably just the coral's mesenterial filaments (its guts basically) that can be expelled outside the polyp during stress, fighting, or feeding (going with stress in this case.)

From the first pic, I didn't see anything wrong. How long had the coral been looking like that? Are any other corals/fish showing signs of stress?

In the first picture if you look at the bottom right polyp it is shriveled. It had also turned blue, looking almost like a bruise. It had been like that for a day or so before I took the picture. And no, no other coral seems to be stressed which is why I feel somewhat clueless
