Help my fish are sick

So after losing my butterfly, I don't really want to lose anymore fish.
So from what I can tell are sick are hippo tang, yellow tang, 3 clownfish , Tomini tang.
Fish that haven't show any symptoms are yellow coris wrasse, flame angel, fire fish, yellow tail damsel, cleaner wrasse.
I have have all this fish for a long time now.

I think they have ick but after looking thorugh other disease it could be brookleynella or flukes maybe???
They have white spots, from those spots It looks like this little white thing is sticking out (kinda like a zit).

What can I do to make sure they survive?
Currently I'm feeding with garlic.

Don't have a qt set up right now, so I would like to be able to treat in the display (reef).
I only have a 20 long, that could work as a qt

You're in the boat I was in. I had many happy fish, but added a Pyramid BF that wound up having ich. Yes, it went through QT, but really didn't see the symptoms until a month or 2 in the display. Eventually almost every fish got sick.

I had to pull all the rock (hadn't been moved in over 9 years) and all corals to buckets. Drained most of the water, only a few inches left in. All this to get all the fish.

I didn't have a large QT, so I bought a couple 20 gallons and set them up. In the 1st few weeks I did loose some of the fish, some died in the 1st & 2nd day. I tried hypo treatment for about 6 weeks, but some of the fish still showed signs of ich. So I raised back up to normal and went with Cupramine this time. I lost a few more, but some of them survived and have now been back in my DT.

DT should be fishless for 6 - 8 weeks and the treatment on the fish should be same. Not fun at all, but really want should be done. I really carefully examine my new fish, if in doubt they wait in QT even longer.

There are some stocking issues (crowding, too many tangs, too many clownfish), but ignoring those, when was the last fish or anything wet added to this tank?

please describe the behavior of the fish? Is this a reef tank or fowlr tank?
