HELP!!!! Sick Bluethroat Trigger

My Bluehthroat Trigger is in bad shape.

His eyes are slightly cloudy, he is very lethargic, his Caudal fin has a tear and his skin is looking bad. Is there anything I can do to save him?

I did a 25% water change. Is there any specific parameters I should be more careful of?
 
I found part of the problem. My Mandarin, whom I hardly ever, EVER, see was dead and rotting. That spiked the Nitrates. I dumped the body. I have to do another water change as well. Is there any medicines that can help him or is it merely a matter of adjusting water chemistry?
 
Do you quarantine all new fish before putting them in your display tank? Anything new added in the past few weeks? How is the breathing rate? Do you see any scratching?

He was doing fine in their for over a month. I think he did scratch himself because I have an eggcrate framework with live rock attached and I think he rubbed up against it.
 
He looks like he is covered with some sort of parasites. As a result, he also seems to be exhibiting fin rot from bacterial infection. You probably will have to treat for both, though which first is the trickier question.
