Help with dying acan

Just like the title says, one of my a and seems to be dying off. He has been in the tank for a few months as was doing great but then I went away for work for a few weeks and let the levels get a little out of wack. I have been back for almost two weeks now and he does not seem to be doing any better. Current levels on the tank are:

Ammonia 0
Nitrates 5
Phosphates .01
Calcium 420
Alkalinity 8.25
Magnesium 1375

Everything else in the tank seems to be doing very well. Is there anything I can do to try and save him?

So I checked the tank out with the lights out and noticed my sea cucumber was crawling all over him. I don't think he was eating him, just trampling him. Would that cause him to stress and lose tissue like this? The cucumber is about 6-7 inches long.
 
I would move the acan I had a hammer coral letting it's tentacles out at nice and I was causing irritation to one of my acan colonies.
 
I had same issue...didnt know what was going on..moved it to lower light and fed it every night..now hes grown 4 new heads
 
well its on the bottom of the tank it a pretty low flow spot not sure where else I can move it to????


I have a few and none are on sand bed and all do ok with a mixture of different flows and light. Try a different place for a few days and see how it goes.
