Help with identification

Hello,

I have a one spot fox face that has something on him I have never seen since entering this hobby 3 years ago.

I have a 180g tank. All my parameters are spot on and my other fish are looking healthy. Can someone help me identify this. Thanks!

How long have you had him? How long has he had the spots? How is his eating, breathing, general behavior (scratching, etc)?

I had him since 1/31/15. I just noticed the spots yesterday evening. He is acting normal. No heavy breathing or scratching yet. He is still eating as well.

I would suggest 2 treatments back to back. So treat wait 5 days, water change then treat again.
