On the ich issue: A WC will do nothing to help your ich problem. But you still need a large WC to deal with the worthless and overdosed med. Your tank has ich, not just a few fish. If some fish are showing ich; sooner or later, they all will. They cannot be treated in your present tank by any method. Have you read the ich stickies above? If you don't have some understanding of ich and its life-cycle; help from anyone will be meaningless.

Like I said earlier and what MrTusk stated, please read all of the ich stickies in this forum. You will need to understand what ich and its life cycle are before you can successfully treat the fish. ALL of your fish need to be treated as they All have ich, whether they show it on the body or not. The ones that are not showing the spots on the body have ich in their gills, where you can't see.

The 50% water change needs to be done immediately. Drug overdose kills fish very fast and you do not have time to ask questions around and hesitating, if you want to save your fish.

i did a 50 percent water change and the spot on the puffer continues to grow and get worse. his spikes are now sticking through the skin. it looks gray bubbly and slimy. i have carbon in the refugium to help remove any left over meds and hes not getting better. ill do anything to save this fish what can i do to save him? thanks so much

Doing a water change has nothing to do with treating for ich. The purpose was to reduce the medication in the water. I stress this again: have you read all of the ich stickies on the top of this forum? To treat ich you need to set up a quarantine tank and apply either copper or hyposalinity. Your puffer sounds like it's in a bad condition and at this moment, its immune system cannot defend the ich attack anymore. You will have to set up the QT, put all of the fish in there, and apply one of the two treatments as soon as possible, or I'm afraid your puffer won't last for a few more days.
