I have an Anemone that looks normal on one side, but deflated and on the other side it is bare. Any ideas, should i isolate it or is it dying?

Sorry to say if this is a new tank that's not good for a nem.
6 mo min for a nem, and even then...
Give us specs;
Time running, water params, lighting, kind of nem.

The tank is right at 6 months, I have had him for about 2 months and he looked great until about a week ago.
I check my water twice a week and other than a ammonia spike about a month ago everything is zero or very close to zero. should i put him in my quarantine tank?
