Help!!!

Ok, here are the stats first. 125g tank with currently Fimby, Pinfish, Javanese Damsel, Sailfin Tang, and a Long-spine Urchin. I JUST ran a water test and the water quality is perfect except for Nitrates around 25-30ppm.

Now the story. Right around the beginning of April, my Pinfish got Pop-eye and recovered within a week. The stock list then was everything listed above plus a Gulf Toadfish, SFE, and Porky Puffer. One month later, my Puffer would not eat for a week and then got some HORRIFIC disease so he was removed from the tank and put in QT. He has remained there ever since and has recovered wondrously. Around a week after the puffer became diseased, I dosed with a small amount of Formalin to prevent any spreading if there was any parasites. No sign of any parasites were ever found. Roughly one week later, my Toadfish randomly died. :sad1: Again, water checked out fine. Then, about a half hour ago, I found my SFE dead. :sad1: He has been eating off and on for a while but the Fimby has just been eating like a pig through nearly all of this. He ate last night a bunch of squid.

Is any of this linked? Water has checked out fine everytime (I even double-checked my work at my LFS), the tank is well-established (1yr. June 5th, it has 100lbs. of LS, and 95lbs. of LR.). I have had NO DEATHS WHATSOEVER the entire time except for my Emperor Snapper. That was due to the fact that he jumped from the tank and fell 62" to the floor, got internal bleeding and died in early November. :sad1: Sorry for the long post, but the remaining fish I truly do love and would absolutely hate for anything to happen to them.
