looks like it might be some type of ricordia... also it looks like it may be dead or dying.

Either Ricordia or Rhodactis. The mouth makes me lean towards Rhodactis. Even Ricordia florida have a more pronounced mouth than what I'm seeing in the picture. The care requirements for both are pretty much the same. Medium levels of light and light to medium flow. Some can be target fed, but it's not necessary.
