there seems to be some type of hole on top of my toadstool, it looks like its trying to split completly in half? is this bad or is it just budding off?

i dont think those split...they just grow a baby on the side...i had one that tore also...right on top in the middle...but i healed & was okay...may be your water

Check your alk. It seems to be a common sign of dangerously slow levels.

 slow = low ... apparently had fat fingers this morning.

Not to hijack but good advice RokieM I think that is my prob my Alk is low and am starting to increase it back up to spec

I have a similar situation with mine. It appears to fragging itself. In fact, the little frag in this picture completely separated, has been attached to a plug, and is now growing out fine. Several other pieces of the mother seem to be doing the same thing. Normal? I don't know, but given that both the parent and babies continue to grow, I'm not worrying about it. The way I see it, it's a constant source of new frags, without the problems inherent with cutting them off myself.

I've got a sarco that self propagates by dropping the edges of the coral when it gets large enough (and holes form in the the cap along the self propagation and elsewhere). Check you ralk (its always good to know where your tanks are anyway), but if it is all in line, I'd not worry too much unless it starts to really degrade.
