A gaping mouth and collapsed tentacles are a couple of signs. Why do you ask?
 
well mine doesn't seem to eat or try to eat. and it's mouth has white lines in the center..it's been in the tank for 3 days now.

the white lines are normal on some. it's mouth is ok. tentacles are not bad. i'd say it's still acclimating to your tank.

Good question, cause so far my anemone has not eaten either but it has only been 24hrs.

All depends on the environment it came from, it's new environment, and the over all health of the anemone.

You really shouldn’t try to feed an anemone until it has acclimated to your system. I’d let it go at least a week prior to trying to feed, and then only very small pieces.
