Ive read numerous posts about how impossible it is to keep an ich free system and the myth that ich is always "present" and that fish always "carry" it and that it is impossible to keep out.
My fish came down with it after not QTing at all. I learned the hard way. QT all fish every single time. I set up a very simply QT system after doing TTM, which works flawlessly and kept the display tank empty for the 72 days recommended. I did run a 24w UV sterilizer for that time just for giggles.
Im at the three week mark with all fish healthy and eating like pigs.
QT every fish, every time.. I do 4 weeks and treat with PraziPro every second transfer while doing TTM.
There are no magical reef safe snake oils. TTM, leave tank alone for a full 70+ days.
