I can't take photos as the spot is so small. There's this one never changing spot on the pectoral fin of my multibar angelfish. It's right on the spine of the pectoral fin. Its like right on the bone. It's not raised or anything it's flush. There's no scratching. The only way to see it is by taking the fish or of the water and looking for it.

He's been in quarantine for a week. I've been doing track transfer. I'm not sure if he's always had it. I specifically went looking for spots and that's how I found it but it's been there for a phew days at least. It doesn't look like ich.
 
If it is persistent and fixed in location and the fish doesn't show any symptoms it may just be a discolored spot. It's for sure not ich.

So I would vote for paranoia. I know exactly how that is - I see spots on my fish all the time (and I know there is very likely ich in the tank), but all turn out to be air bubbles, dust, or sometimes coral stings.

You know that you have ich in your tank? How do your first not get it?
 
If it's still there it isn't ich.
Could be a small Lymphocystis nodule or it could be nothing.
In any case, if the fish doesn't seem bothered and it doesn't change over a week I would not be too worried and just keep an eye on it.
