In tank for three weeks Naso Tang is Sick

Please Help

My Naso tang has been in the tank for three plus weeks. About 5 days ago he stopped eating and is slow moving. He has a salt like grain on his fin (see pic) and some white spots. WHat is it (Ick) and can I treat in my main tank (Fish only) I can not catch him!
Every disease I have read about does not match what I see can anyone tell me what to treat him with and for how long or can I just leave him alone?

There is no picture, but if it is ick, and your tank is fish only, then it's pretty easy to treat. (As long as you don't have any sharks or rays).

Slowly reduce the salinity to 1.009-1.010 and keep it there for at least 14 days. After that, raise it back to normal at the rate of .001-.002 a day.

Also, slowly increasing the temperature into the mid 80's will speed things up.
