Is my euphyllia dying?

This guy has been in my tank for about a month, and seemed to be doing well until I came down this morning. He's been recessed all day (usually only sucks up for an hour or two) and the second head appears to be rotting. I'm not sure if there's anything I can do to salvage. Should I dip it? :confused:

The attachment 'hammer' shows the coral a week ago, and what it looked like yesterday.

I've attached all metrics about the tank in screenshots from the controller... it's a 72 gal bowfront with a 20 gal sump and a 5 gallon fuge. HALP! :bounce2:

Sorry - the tank was cycling 3-4 months ago when I signed up for the site. The tank has been cycled and stable for some time now... I'll update my sig.
 
Last posts were cycling in September, so hopefully its done now. As for that frag, yea its dying. What lights are you using?
 
I have 2x gen 4 radions. Screenshots of the profile I'm using are attached.

Can I frag it with regular large curved blade frag tool, or do I need a dremel?

Just finished the cut... There's a black ring around the outside of the 'good' head, so I think I might be too late. Thanks for the quick reply all!
