How long has it been like this?

i bought it mon., (it looked great after i acclamated it and placed it in my tank!), and it moved from the spot i placed it mon. night in the middle of the night. tues. i moved it back and it moved again but late tues. it started balling up like this :( weds. i did a 20% water change and moved some rock around and placed it again, it acted like it was starting to open up again then it moved and has looked like this ever since :(

It doesn't look dead to me. I would just leave it alone for now since you probably stressed it out by moving it again.

The anemone will move to where it wants to so you could end up killing it if you try to put it where you want it.

BTW If it was dead/dying it would look like it were disintegrating. Similar to the ones on this page but worse.

i guess it looks more like tearing than disintegrating. being a newbie my first thought was maybe it's splitting!? i don't know what that would look like so i'm not sure!? this SUCKS!!

The pics look like there are numerous tears on the foot. Did you try to put the anem in between some rocks? That might have caused the damage. This species puts its foot into sand. Only other thought would be maybe something has decided to try eating the anem. Sorry to say, but even with ideal tank params this will be a difficult recovery.
