Is this ich?

I noticed my royal gramma has a few white spots on its tail recently however it is swimming and eating normal. Can someone please tell me if it's ich? Thanks

Yup looks like ich.
How old is your tank?
What was the last thing you added and when?
Do you have a Quarantine Tank? And was everything QT'd before adding to the Display Tank?

Tank is 5 months old. I suspected it was the yellow clown goby I got 2 months ago however she was gone a few days after I got her. Everything was fine until now. Unfortunately I don't have a QT.

Any suggestions for treatments? I got a bottle of herbtana and wonder if it will work.

Read the stickies they will tell you what works and what are myths.
Herbtana and any bottle solution claiming to cure ich (besides copper treatment) won't work.

Since you have corals in your tank, you are going to have to set up a QT
 
Grammas are very prone to ich. How long has he gramma been in the tank? If it were me, and the gramma was well established, I'd give it a week or so to see if it fights it off. If it starts looking worse then it's time for treatment and a fallow tank.
 
You will need a QT container and treat the fish with cupramine or something similar.

You will also need to leave the system fishless for about 3 months. You dont do that the fish will probably die.

Believe me I learned the hard way. You need to QT everything, even corals.
 
There is no reef safe treatment for ich.

Here are possibilities that might happen:

1) Do nothing, or treat with something claiming to be reef safe: Fish immune system might fight it off with good diet and environment and water quality. But it will always be there, pestering your fish... possibly one day getting out of hand and killing them.

2) Do nothing, or treat with something claiming to be reef safe: Ich will overcome their immune systems and wipe out your fish.

3) Remove all fish, let tank sit without any fish for 90 days. Treat Fish with any non-reef safe treatment in a hospital tank. Tank Transfer(some have reported failures-possibility from cross contamination), Chloroquine Phosphate (My personal choice-can't be used with pipe fish, sea horses, some wrasse), Copper/cupermine (tough on fish, not recommended) Hyposalinity (works well and is not harmful to the fish as long as you are diligent about keeping the water topped off and follow the instructions on how to do it, lowering it gradually, then raising it back up even more gradually). Always treat new fish for ich before being added to the tank. -No more ich in tank.
 
My Royal Gramma had Ich once and I was able to treat him with Cupramine in my QT. Today he's healthy as could be. But you better start treating him ASAP.
